1. I agree to give my puppy flea and heartworm treatments. I also agree to provide general health care, including the completion of immunizations plus yearly boosters and prompt veterinarian care for any illness requiring medication or surgery.

2. 72 HOUR HEALTH GUARANTEE:  I agree that I received my puppy in sound health and with a veterinarian record from the College Kitty veterinarian. My puppy was transferred to me in good faith as a healthy and well cared for animal. I AGREE THAT I WILL TAKE MY PUPPY TO MY VETERINARIAN WITHIN 72 HOURS. I UNDERSTAND THAT FAILURE TO DO SO WILL RENDER THIS CONTRACT NULL AND VOID.  If my puppy is found to have a serious or life threatening problem, I must notify College Kitty immediately. Upon return of my puppy, College Kitty will issue a refund. I UNDERSTAND THAT COLLEGE KITTY IS NOT RESPONSIBLE FOR MY VETERINARIAN BILLS.

3. I understand that my puppy is guaranteed against congenital defects for one year. Should my puppy die within that time as a result of a congenital defect, the purchase price of my puppy will be refunded ONLY after my veterinarian's certificate is provided as proof of cause of death. I UNDERSTAND THAT COLLEGE KITTY IS NOT RESPONSIBLE FOR MY VETERINARIAN BILLS.

4. Seller has the unconditional right to reclaim puppy/dog without refund for any breach of contract.

I agree to the terms of this contract. I understand that I am signing a binding contract.

Your puppy/dog is welcome back to College Kitty if you can no longer care for your puppy/dog or if you change your mind about puppy/dog ownership. Please keep in mind that we will be accepting your puppy/dog back with no refund of purchase price or promise of another puppy/dog.
